How do I sign up for Memorial Hospital Patient Portal?

Provide your email address upon registration at Patient Access and a representative will be happy to help you set it up.

What do I need to sign up for Patient Portal?

Enrolling in Patient Portal will take less than fifteen minutes. You will need to have proper identification with you to enroll. Acceptable forms of ID include:

  • Driver's license
  • State issued ID
  • U.S. Military ID
  • Memorial FOS Staff ID
  • Passport
  • Marriage license*
  • Divorce decree*
  • Adoption papers*
  • Social security card*
  • Birth certificate*
  • Power of Attorney
  • * must be accompanied with a valid picture ID

What if I can't remember my password?

Enter the information you provided while setting up your account here:

  • Username
  • Hospital PIN number
  • Email address

Will Memorial Hospital share my email address?

No, your email address is only used for CONFIDENTIAL COMMUNICATION about your medical history with Yakima Valley Memorial Hospital.

Why can't I see my lab results?

Lab results are visible once they have been reviewed by the doctor. If they are not visible, your lab results may not be final yet. Please call the office if you have not received notification regarding your lab results within 2 weeks of your visit.

Why can't I see all of the information in my record?

The portal does not allow all portions of the medical record to be visible. Tests, vaccines and care administered prior to registration with our portal may not be visible.

Does the portal provide medical emergency services?

No, Our Patient Portal is designed to help you monitor your personal medical records. It does not provide any emergency medical service. If you have a medical emergency, please call 911.

Will Patient Portal allow me to make online appointment or cancellation requests?

No. Making or cancelling an appointment cannot be done through Patient Portal at this time.

Can I enroll my child in Memorial Hospital Patient Portal?

At this time, minors under the age of 18 years cannot enroll for access to their online account.

How do I get technical help accessing Memorial Hospital Patient Portal?

Call 509-972-1155, Monday – Friday, 8am – 5pm, if you have trouble logging in or have question on how to use your Patient Portal.
